chore(tui): remove plan-label comments and session-work identifiers - #130

Rename wave-numbered test files to descriptive names, strip C/F/P/E label prefixes and divider rules from comments, and drop internal review-audit references (judge-N, .ux-review/, Round-4 wave-1).
