Merges raine/claude-code-proxy at 4b5c058a (v0.1.44) into main. Done by hand after the 10-08 automated sync stopped on a CHANGELOG.md conflict.

Conflicts: CHANGELOG.md only. Kept the fork Unreleased section above upstream's v0.1.44 section.

Fork decisions: no upstream change touched them. I checked the src diff for fable, astra, kiro, sonnet, opus, speed, and service_tier: no matches.

Checks: just check-ci passes locally on Rust 1.99.

When Codex keeps finishing a turn with no output, the proxy already
retries up to 10 times (~3 min) before answering 503. Claude Code then
retries that 503 itself, up to 10 more times, and each of those repeats
the proxy's full retry loop. A deterministic empty completion therefore
holds the request for roughly half an hour (~100 upstream calls) before
it fails.

This reproduces reliably with background subagents: after a subagent
hands its report back, GPT models often have nothing left to say, so
the follow-up turn completes empty every time and the agent stays
"running" until it is killed.

Mark the exhausted-empty 503 with `x-should-retry: false`, the same
signal usage_limit_response already uses. Claude Code's shouldRetry
honors it, so the request fails once, after the proxy's own retries.
