Translation of php/doc-en#320 (commit 7c915c0): new "Error handling" chapter and dedicated throw, try-catch and finally pages, replacing the former errors and exceptions pages, with all cross-references updated. EN-Revision updated.
The DOM, PDO and SNMP exception pages were also behind on unrelated markup cleanups (skipped revcheck commits), which are carried over here.
