Merge ordering: Without #15116 (fixes #12170), a lone streamed rate_limit_event for an account in the new probeFailed state is published as the complete window list. This PR makes that state reachable for this account shape (previously locked at unsupported), so it should land together with or after #15116. No code from #15116 is included here.

Problem

A Claude account whose get_usage probe returns rate_limits_available: true with rate_limits: null is classified unsupported. That reason hides the account from Usage → Limits with no bar and no notice, and applyUsageLimitsUpdate ignores streamed rate_limit_event windows for it, so it never recovers. The CLI sends this shape when its own usage fetch fails, so a subscription account (a Team account in the issue's capture) disappears from Limits. Expected: only accounts that can never report limits (rate_limits_available: false) are unsupported.

Change

claudeUsageResponseToLimits in apps/server/src/provider/claudeUsageLimits.ts now returns unsupported only for rate_limits_available: false. The true + rate_limits: null shape returns probeFailed. Limits then shows "Could not read limits.", streamed windows are accepted, and the next successful probe replaces the state. No contract or client change was needed: the lock and the silent omission both come from the unsupported reason.

Scope and approval

Fixes #13053, a triaged bug (labels bug, accepted). The maintainer triage lists this exact change as its first suggested fix: reserve unsupported for accounts that can never report subscription windows and map rate_limits_available: true with missing rate_limits to probeFailed. A commenter captured the real payload from a Team account: {"subscription_type":"team","rate_limits_available":true,"rate_limits":null}. #15364 reports the same defect.

One underlying problem: the probe's classification. Separate and not touched here: #12170 (sparse rate_limit_event published as complete, handled by #15116), the CLAUDE_CODE_OAUTH_TOKEN setup-token case (which really reports rate_limits_available: false), and the triage's optional items (logging the get_usage shape, a UI notice for unsupported).

Verification

  • New test "reports a subscription account whose usage read came back empty as a failed probe" in claudeUsageLimits.test.ts. On main it fails (received unsupported, expected probeFailed); with the change it passes. The existing "API key and Bedrock accounts as unsupported" test still passes.
  • In apps/server: vp test run src/provider/claudeUsageLimits.test.ts → 8 passed (1 failed, 7 passed before the fix). vp test run src/provider/ClaudeCapabilitiesProbe.test.ts src/provider/ProviderRegistry.test.ts src/provider/claudeUsageLimits.test.ts → 76 passed.
  • In packages/provider-core: vp test run src/server/usageLimits.test.ts src/server/managedProvider.test.ts → 17 passed.
  • tsc --noEmit -p . in apps/server: 0 errors. vp fmt clean.
  • Not checked: a live Team or second Claude account, and the Limits tab in a running client. The payload shape comes from the issue; the tests drive the mapper directly. No UI change, so no screenshots.

Approvability

Verdict: Approved at 6be3548

Macroscope's review found this PR approvable — This is a focused bug fix that reclassifies one transient Claude usage response so accounts remain visible and can recover from streamed or subsequent usage data. The existing unsupported-account behavior remains intact, and the new case is covered by a targeted unit test.

📝 Walkthrough

Claude usage response conversion now distinguishes an explicitly unsupported response from a response with available rate limits but no rate-limits payload. A test verifies the latter returns probeFailed with empty windows and no overage-included name.

Changes: Claude usage limits

The conversion returns unsupported when rate_limits_available is false and probeFailed when availability is true but rate_limits is absent. A test checks the probeFailed result, empty windows, and absence of an overage-included name.

Merge Risk: 🔵 Low · up to 6be35

A failed Claude usage read followed by one streamed event can make the Limits view appear complete while omitting other windows. Preserve or safely merge partial state before merging.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 6be35

The change corrects account usage reporting without demonstrating expanded access or weakened authorization. However, deployed alone, a single streamed window can clear the failed-read warning while other limits remain unknown. The companion recovery fix therefore matters.

Retained concerns

  • Low · reliability · observed: Without the companion sparse-recovery protection, an empty probeFailed snapshot accepts one streamed window and becomes an apparently available snapshot without the failed-read notice. Other windows remain unknown until further events or a successful probe. The PR newly enables this transition for true-availability/null-payload responses, creating a bounded failure-containment and rollout-ordering concern rather than an authorization finding.
Security review details

Security Blast Radius

  • inferred — The demonstrated effect is usage reporting for Claude accounts returning the affected response shape. Native events target the owning provider-instance snapshot, and the external usage caller preserves the selected source account. No inspected path adds cross-instance selection, credentials, or service-wide authority.

Trust Boundaries and Controls

  • observed — Probe publication detects differing known account emails or workspace IDs before reusing old limits. Event processing checks current query ownership, and query exit or close-timeout cleanup drops ownership. These controls constrain stale updates, but do not establish account-generation binding for a query that remains live across an account reprobe.

Resilience and Maintainability Implications

  • inferred — The recovery defect weakens failure visibility rather than demonstrating weakened access enforcement: the first sparse window removes the transient-failure marker, and presentation then returns no failed-read notice. This bounds the observed concern to incomplete usage state and diagnostic containment.

Explanation

The change satisfies #13053 for the reported payload: rate_limits_available: true with rate_limits: null now maps to probeFailed, and the regression test covers this case. The change does not satisfy directly linked #12170. It does not change applyUsageLimitsUpdate or preserve the unavailable marker when a streamed window merges with a failed-probe snapshot. A lone event can therefore still publish a partial window list as complete.

… unsupported

The Claude usage probe marked an account `unsupported` whenever `get_usage`
returned no `rate_limits`, including when `rate_limits_available` was true.
The CLI sends that shape when its own usage fetch fails, so a subscription
account was locked out of Limits with no bar and no notice, and streamed
rate_limit_event windows were discarded. Only `rate_limits_available: false`
is now `unsupported`; the other shape is `probeFailed`, which shows
"Could not read limits." and recovers on the next probe or streamed window.
